Activity Description

Intended Audience

Physicians who treat and manage patients with post-surgical skin/soft tissue infections, catheter-related bloodstream infections, and/or ventilator associated pneumonia in a hospital setting, and want to be engaged in a performance improvement activity to identify and reduce CA-MRSA occurrences.

Elements of Competence

This activity has been designed to change learner knowledge, competence, and performance focusing on the American Board of Medical Specialties areas of patient care, medical knowledge, systems-based practice and practice-based learning and improvement.

Statement of Need

Over the past 40 years, HA-MRSA has become endemic in most US hospitals and in hospitals worldwide, and is a well-documented problem in healthcare settings. By contrast, CA-MRSA has emerged as a serious problem only recently, beginning in the 1990s. Epidemiological and molecular biological evidence have shown that CA-MRSA developed outside of healthcare settings and is independent of HA-MRSA. CA-MRSA has now emerged as a nosocomial pathogen, causing serious infections in hospitalized patients, including skin and soft tissue infections (SSTIs) and, to a lesser extent, severe forms of invasive disease, including BSIs, endocarditis, pneumonia, pyomyositis, necrotizing fasciitis, toxic shock syndrome, and bone and joint infections, as well as the recent report of a brain abscess caused by the USA300 strain of CA-MRSA.

Objectives

At the conclusion of this activity, participants will be able to:

  1. Identify and reduce risk factors for MRSA surgical site infections in hospitalized patients, including the potential for CA-MRSA as a pathogen
  2. Identify and reduce risk factors for MRSA catheter-related bloodstream infections, including the potential risk of CA-MRSA
  3. Identify and reduce risk factors for MRSA ventilator-associated pneumonia, including the risk for CA-MRSA as a potential pathogen

Performance Improvement Process

The three stages of the PI activity are:

  • Stage A: Learning from your current practice performance assessment.
  • Stage B: Learning from the application of your performance improvement plan.
  • Stage C: Learning from the evaluation of your performance improvement effort.

Activity Eligibility, Enrollment and Expiration Information

Participants are eligible to enroll in this activity if they can commit to the following:

Stage A: 1 day to 1 month in length

  • Complete Learning from Self-assessmentTM(LSA)
  • Select most appropriate activity measures based on results of LSA (NOTE: 4 measures will be required for every participant, and 3 additional measures are available to be selected as optional.)
  • Complete retrospective chart review of 25* patients who meet selected performance measures criteria (start with patients for whom you are/have been responsible, move to your clinic/unit patients, then organizational)
  • Complete Learning Log A (includes implementation plan)

Stage B: 3 month minimum in length(Stage B will ONLY be accessible after completion of the required documents in Stage A)

  • Engage in educational activities, tools, and resources to facilitate change
  • Implement practice changes for a period of time (a minimum of 3 months) allowing for the maximum number of appropriate MRSA patients in order to re-measure in Stage C (start with patients for whom you are/have been responsible, move to your clinic/unit patients, then organizational*)
  • Not less than 6 weeks into Stage B, and after completing at least one of the educational components, complete a ‘progress check’ chart review of 10 patients who meet selected measures criteria
  • Complete Learning Log B

Stage C: 1 day to 1 month in length (Stage C will ONLY be accessible after a minimum of 3 months of engagement in Stage B)

  • Complete Learning Self-assessment
  • Complete chart review of 25* patients seen in Stage B who meet selected measures criteria(start with patients for whom you are/have been responsible, move to your clinic/unit patients, then organizational)
  • Complete Learning Log C

Important Dates:

  • Enrollment begins: June 24, 2011
  • Enrollment ends: March 21, 2013
